REFERRAL & MARKETING SPECIALIST (Full Time)
New position – Responsible for:

  • Developing and maintaining a broad scope of referral resources
  • Initiating service coordination with the Intake Department.
  • Calling on physicians & case management staff.
  • Case conferencing with managed care case managers on mutual clients
  • Keeping Director of Intake informed of volume and flow of referrals
  • Participating in marketing initiatives, proposals and other projects

Requirements:  Current MA RN license; BSN preferred.  BA/BS Degree in Marketing, PR, Health Care or Business Management or significant relative experience in a similar position with demonstrated competence in expanding the number of referral resources and increasing the number of referrals; 3 -5 years of directly related home care nursing experience including an up-to-date knowledge of reimbursement for home health care by Medicare/Medicaid/private insurances/managed care organizations; excellent communication and customer service skills; and computer literacy. Current Driver’s license and reliable automobile.

WEEKEND NURSING SUPERVISOR
Experienced nurse to influence and direct staff to perform to their maximum potential through communication, education and performance management, who clearly defines assignments, and who fosters and maintains an environment of open communication and mutual respect.

  • 1 weekend (Saturday & Sunday) in a 6 week rotation
  • 3 holidays per year
  • Includes working from approximately 3 to 8 p.m. the evening prior to your assigned weekend or holiday to prepare employee assignments
  • Extended Hours Supervisor is responsible for on-call beeper coverage from 4:30 p.m. the evening prior through 8 a.m. on the day following your assigned weekend or holiday

Requirements:  Current MA RN license, BSN or equivalent, at least 3 years of directly related experience, including two in Community Health.

COMMUNITY HEALTH  NURSE (Full Time # or Per Diem ## )

  • Responsibilities include but are not limited to the promotion of health maintenance of individuals, families and the community through assessment, teaching, counseling and appropriate preventive and rehabilitative nursing measures, working with Agency policies and under the ANA Code of Ethics.
  • Performs similar or related duties as assigned/required.

Requirements: Graduate of NLN nursing program approved by the Massachusetts Board of Registration in Nursing (BSN preferred), current Massachusetts RN license plus one year’s  experience in community health nursing and/or one year’s recent acute care medical/surgical nursing experience, or current LPN license with at least 2 years home care or acute medical/surgical experience. IV certification and bilingual skills a plus. Must have current Massachusetts’s driver’s license and a reliable automobile. Must possess gross motor skills in order to get in/out of vehicle, climb stairs, carry community health nursing bag, etc.  Fine Motor skills to accomplish skilled nursing care and required documentation.

# Includes every third weekend rotation plus 1 major winter, 1 major summer, and 1 minor holiday
## Includes working one weekend in a 6-week rotation plus 1 major winter, 1 major summer and 1 minor holiday.

PHYSICAL THERAPIST – Per Visit (Full or Part Time )
  • Plans, organizes, and conducts physical therapy program in the patient's home to facilitate Diversified adult caseload (Orthopedics, Neuro, and Geriatrics)
  • Tablet PC documentation (Misys)
  • Visit rate compensation
  • Mileage reimbursement
